How to define an outcome and record whether it was realized

Short answer: Open Outcomes → New outcome (or Add an outcome on an objective or initiative) with its level, baseline, expected result, measure, target and expected date. When it is time, open it and choose Record decision with the realization and its basis. Completing a project never realizes an outcome.
